In the UK, the procedure of reporting your illness is a bit different from how it works Poland – here’s how it looks like step by step:
Illness lasting up to 7 days – self-certification
If you’re ill for up to 7 calendar days (including weekends), you don’t need to see a doctor. All you need to do is:

inform your employer that you’re unwell,

complete a self-certification form (form SC2).
You can:

get the form from your HR department, or
-
download it from the NHS website: Self-certification SC2.
This declaration is recognised as a valid sick note.
Illness lasting more than 7 days – you need a doctor’s certificate (fit note)
If your sickness lasts longer than 7 days, you must obtain an official medical certificate from a doctor – the so-called fit note.
This document states whether:

you are completely unfit for work, or
-
you can work with certain adjustments (e.g. remote work, avoiding heavy lifting).
How to get a fit note from your GP (family doctor)
To obtain a fit note:

call or book an appointment online with your NHS surgery,

your GP may issue the certificate after a phone call, video consultation, or in-person visit,
-
the document can be sent by text, email, or printed for collection.
Remember, you must be registered with a GP practice to receive a fit note under the NHS.
Alternative: Polish doctor in the UK
You don’t have to rely solely on your GP. You can also get a fit note from a Polish private clinic in the UK. Many patients choose this option because:

communication is easier and waiting times are shorter,

online consultations are often available.
Important: The doctor must be registered with the GMC (General Medical Council) for the certificate to be valid and accepted by your employer.
Submitting your sick note to your employer
Once you receive your fit note, submit it to your HR department. You can do this:

by sending a scan or photo via email.
You don’t need to provide medical details – the document confirming your inability to work is enough.
Extending your sick leave
If your fit note is about to expire but you’re still unwell:

contact your doctor before your current fit note runs out,

Your employer cannot require you to return to work without a valid certificate.
Sick leave and remote work
